Basic Requirements of a Shoreland Alteration Permit:
1. The smallest amount of bare ground shall be exposed for as short a time as possible.2. Temporary ground cover shall be used, and permanent cover shall be established and
maintained.
3. Diversions, sediment basins, and other methods shall be used to minimize runoff and
erosion.
4. Fill shall be stabilized.
Wetland !nformation:
Notice: You are responsible for complying with State and Federal Laws concerning construction near
or on wetlands, lakes, and streams. Wetlands that are not associated with open water can be difficult
to identify. Failure to comply may result in removal or modification of construction that violates the law
or other penalties or costs. For more information, visit the Deparlment of Natural Resources wefland

Resources Service Center.
Buffer Zone Tree and Shrub Removal:
Tree and shrub removal is prohibited within the Shoreland Buffer Zone (35' landward of the OHWM).
Trees and shrubs less than six inches in diameter may be removed within the VAC. Exception for
trees that are dead, dying or diseased (recommend photo documentation).

Project Details
Provide a brief description of the proposed shoreland alteration project. lnclude list of materials that
will be used for sediment and erosion control during construction, and describe how the site will be
permanently stabilized post-construction. The attached plot plan must include detailed locations of
all erosion control materials. Erosion Control - may include but not limited to: silt fence, straw
bales, and erosion control mats
