The specifiwtions and/or plans for th(s plumbing produd have been reviewed and determined to comply with chapters SPS
382 through 384,Wisconsin Administrative Code,and Chapters 145 and 160,Wisconsin Statutes.
The Oepartment hereby issues an approval based on[he Wisconsin Sta[utes and the Wisconsin Administrative Code. This
approval is valid untii the end of May 2027.
This approval is contingent upon compliance with the following stipulation(5):
1. A copy of this approval letter shall be submitted with ali plans using the Mound Component Manual for POWTS
(Version 2.1),(May 2022-2027)_
Plans submitted without a copy of this approval letter may be denied.
2. This approval recognizes that POWTS systems designed,installed and maintained in accordance with this manual will
provide treatment and dispersal of domestic wastewater that is acceptable in the contexi of ch.383 Wis.Adm.Code.
3. Systems installed in accordance with[his POWTS Component Manual shall use wastewater tanks approved hy the
department. If a given tank is approved and meets the published specifications contained in the manual,then
redundant approval of the tank is not required. The installation shall not�ompromise the structural integrity of the
tank.
4. Sys[ems installed in accordance with this POWTS Component Manual shall be installed,maintained and used in strict
accordance with[he manufac[urer's published instructions,Chapters 381387�s.Adm.Code and this product
approval.If there is a conflic[between the manufacturer's instructions and the Wis.Adm_Code orthis Plum6ing
Produc[Approval,then the Wis.Adm.Code and this Plumbing Product Approval shall take precedence.
S. Complete operation and maintenance instructiors POUVTS systems designed in accordance with this manual shall he
provided to each system owner and remain onsi[e.
6. Systems designed in accordance with this manual shall be installed by persons holding the proper license or
registration in accordance with Wis.Stats.4 145.
� 7. Drain,waste and ven[piping used to ins[all these sys[ems shall conform to s.SPS 384.30(1�,(2)and (3)W is.Adm.

8. Cleanouts shall be installed in drain piping associated with the ins[altation of these systems in accordance wkh s.SPS
38235 Wis.Adm,Code.
9. Commercial food processing,food production,food service,res[aurants,taverns and similar esta6lishments which
may generate greases,fats,oils or similar substances;shall have statrapproved grease interceptors installed
upstream of POWTS rystems designed in accordance with this manual in a�cordance with s.SPS 35234 Wis.Adm.
Code.
10. DSPS POWIS plan approval shall he obtained from the departrnent's Private Sewage Section,or the appropriate agent
county,for:
a. each installa[ion of POWiS sys[ems designed in accordance with this manual;and
b_ high-s[rength and/or commercial POWfS rys[ertu designed in accordance with this manual.
11. A sankary permit shatl be obtained,in accordance wi[h s.SPS 383.21 Wis.Adm.Code,from the counry,or other lacal
authority having jurisdiction,for each proposed installation of rystems designed in acwrdance with this manual.
12. A complete and accepta6le soil evaluation report,confarming to s.SPS 385.40 Wis.Adm.Code,shall be performed for
all proposed rystems designed in accordance with this manual.

SITF. REQUIREMENTS
• A full size copy of the approved plans, specifications, and this letter shail be on-site during construction and open to inspection
by authorized representatives of the Department, which may include local inspectors_ A Deparlment electronic stamp and
signature shall be on the plans which are used at the job site for construction.
The following conditions shall be met during construction or installatinn and prior to occupancy or use:

• Preserve dispersal area prior and during construction to avoid disturbance, compacrion and use of the site.
• With new construction; it is recommended not to activate the putnp in the dose tank until the tanks are pumped prior to
homeowner occupancy.
• W'astewater generated from contractors deaning of equipment and tools and/or left o��er construction products shall not be
discharged into the drains discharging to the private onsite wastewater treatment system (POWTS). Waste generated shall be
properiy disposed of on-site or off site.
. Any tall grasses, leaves and shrubs shall be cut short and remo�ed prior to tilling the surface for installation to prevent mattin;�
under the dispersal area. All loose organic material to be removed from POWTS Dispersal Area.
• Divert surface water from all POWTS Areas.
• Prior to construcfion of the dispersal area, check the moisture content of the soil to a depth of 8 inches. Smearing and
compacting of we1 soii will result in reducing the infilhation capacity of the soil. Proper soil moisture content can be determined
' by rolIing a soil sample between the hands.If it rolls into a 1/4-inch wire,the site is too wet to prepare_If it crumbles,site
preparation can proceed. If the site is too wet to prepare,do not proceed until it dries.
• All piping shall conform to SPS Table 3R4.30-3 and SPS Table 38430-5
• Insulate building sewer beyond 30 feet per SPS 38230(11){c)
• VVell setbacks to meet chs.NR 811 &812
• Tank Installation to follow all manufacture's recommendations.
• Verify property line(s)prior to installation.
• Pump Floats to be set and verified per approved plan. Any changes may result in pump resizing to meet'fDH and GPM
Specifications.
• 4reas that arr uccupied with rock tragments,tree roots,stumps and boulders reduce the amount of soil available for proper
treatment. If no other site is available,trees in the basal area of the mound must be cut off at ground level. A larger fill area is
necessary when any of the above conditions are encountered,to provide sufficient infiltrative area.
OWNER 1ZESPONSIBILITIES
. The current owner,and each subsequcnt owner,shall receive a copy of this letter including instructions relating to proper use
and maintenance of the system_ Owners shall receive a copy of the apprapriate operation and maintenance manual and/or
owner's manual for the POWTS described in this approval and Wis.Admin.Code§ SPS 383.54(1).
• In the event this soil absorption system or any of its component parts malfunctions so as to create a health hazard,the property
owner must follow the contingency plan as described in the approved plans.
The submittal described above has been reviewed for conformance with applicable Wisconsin Administrative Codes and Wisconsin
Statutes. The submittal has been CONDITIONALLY APPROVED. This system is to be constructed and located in accordance with
the enclosed approved plans and with any component manual(s)referenced above. The owner,as defined in chapter 101.01(10),
Wisconsin Statutes,is responsible for compliance with all code requirements.
No person may engage in or w�ork at plumbing in the state unless licensed to do so by the Deparhnent per s.145.06,stats.
All permits required by the state or the local municipality shall be obtained prior to commencement of
construction/i nstal lation/operation.
In granting this approval,the Division of Industry Services reser�%es the right to require changes or additions,should conditions arise
making them necessary for code compliance. As per state stats 101.12(2),nothing in this review shall relieve the designer of the
responsibility for designing a safe building,structure,or component. The Division does not take responsibility tior the design or
construction of the reviewed items.

General
� TMs system shall be operated in accordance with SPS 382-84 Wis.Adm.Code,and shall maintained in accordance with ifs'compq�nt
manuals[560.70691-P(N.Ot/01,R.11/72),SSWMP Pubfication 9.6(01/87),arM Pressure Distribution ComponeM Manual Ver.2.0 SBD-
10706-P(N.01l01,R.10/12)]and Iocal or state rules pertainin9 to system maintenance and mainlenance reporting.
No one should ever enter a septic ar pump tank since dangerous gases may be prrsent that cauld cause death.
Septic and pump tank abandonment shall be in accordance with SPS 383.33,Wis.Adm.Code when the tanks are no longer used as
POWTS components.
Septic or pump lank manhole risers,access risers and covers should 6e inspected Tor water tigMness and soundness. Access openings
used for serv�ce and assessment shall be seaied watertight upon the completion of service. Any opening deemed unsound,defedive,or
subject to failure must be replaced. E�osed access openings greater than 8-inches in diameter shall be secured by an efteclive locking
device to preve�ri accidental or unauthorrzed enUy into a tank or component
SepticTank
The sepGc Yank shall be maintained by an individual cedified to service sepOc tanks under s.281.48,Stats. The contents of the septic
tank shall be disposed of in accordance with NR 113,bY�s.Adm.Code. The operating condition of the septic tank and outlet filter shall be
assessed at least once every 3 years by inspection.
The outlet 51ter shall be cleaned as necessary to ensu2 proper operaUon. The filter cartddge should no[be removed unless provisions
are made to retain solids in the taMc that may slough oH the fiker when removed from iLs enclosure. IiMe filler is equipped with an alann,the
filter shall be serviced if the alartn is activated continuousty. Intermitlentfil[er alartns may indicate sufge flows or an impending continuous
alarm.
The sep6c tank shall have iCs canlents`emoved when the volume of sludge and scum in the tank exceeds 1/3 the Fquid volume of the
tank If the cnntentr of the tank are not removed at the Gme of a[rienn'ral assessment,maintenance personnel shall advise the owner of

The addition of bialogical or chemical addNves to enhance septic tank pertormance is generaliy not required. However,if such produUs
are used they shall be approved tor septic f2nk�se by ihe DepaMmnt of Commerce.
Pumo Tank
The pump(dosing)tank shall be inspeded at least once every 3 years. All switches,alartns,and pumps shall be tested lo vecify proper
operation. Ifan effluent filter is inslalled within the tank it shall be inspected and senriced as necessary.
Mou�d and Pressure Distributlon Svstem
No trees or shrubs shoutd be planted on ihe mound. Plantings may be made around fhe mound's perimeter,and the maund shall be
seeded and mulched as necessary to prevent erosion aMl to provide some proteclion from fras[penetrafion. Traffic(other than for
vegetaUve maiMenance)on the mound is not recommended since soil wmpaction may hinder aeration of ihe infiltralive suiface wilhin the
mound and snow compaction irrthe winter w1ll promMe frosl penetration. Cold weather installa[ions(Odaber-February)diclate fhal the
mound be heaviy mulched as proleIXion finm freezing.

The pressure d�stnbution sys[em is provided with a Flushing poirt at the end of each lateral,and it is recommended that each Iaterai be
flushed of accumulated solids at Ieast once every 18 months. When a pressure test is pertormed it shauld be compared to the inflial test
when Ne system was installed to determine if orifice clogging M1as occurtc�d and H orfice cleaning is required to maintain equal distri6ution
xnlhin the dispelsal cell.
Observation pipes wilhin the dispersal ceA shall be checked for efiluent ponding. Ponding levels shall be reported to fhe owner,and any
levels a6ove fi inches considered as an impentlng hydraulic failure requiring addRional,more frequent moniWring.
Continqencv Plan
If the septic tank or any of its wmponents become detective the tank or component shall be repaired or replaced to keep the system in
proper operati�condition.
If the dosing tank,pump,pump controls,alartn or ralated wiring becomes defective the de(ective component(s)shall be immedia�ely
repaired or replaced with a component oF fhe same or equal pertortnance.
If the mound component fals to accept wastewater or begins to discharge wastewater to the ground surface,it will be repaired or
replaced in its'present locafian by increasing basal area t/toe leakage occurs or by remo�ing biologically clogged ahsoiption and dispersal
media,and related piping,and repladng said componenis as deemed necessary to bring itte system Inlo proper operating conddion.
