Show lot dimensions and indicate north.
Draw the location, size, and dimensions of each structure on the property.
Indicate new structure. Show driveway access.
Draw the location of the septic system and distance to structure.
Show the distance to the shoreland of the waterbody. Show location of any wetland.
Show the distance from the structure to road centerline, lot lines, wetland, septic system
(drainfield and/or tank).
Shoreland vegetative buffer:
The shoreline area is to. be fully vegetated to a depth of 35 feet from the shore of any waterbody. The
exception is an access corridor of 30 feet within any 100 feet of shoreline. Indicate on the above
drawing the location of the access corridor. Owner's signature is required on the appropriate attached
Shoreland Protection Agreement. This permit cannot be issued without a signed Shoreland Protection
Agreement. If the 35 foot buffer is fully vegetative, sign the Shoreline Protection Agreement #1.
If any part of the 35 foot buffer is not in compliance with vegetative requirements, sign the Shoreline
Protection Agreement #2 — Non -Intact.
Please submit a shoreline photo to verify buffer status.
Notice:. Land Use Permits expire one year from issue date.

compliance with the requirements of the Sawyer County Zoning Ordinance and the laws and regulations of the State of

The owner(s) hereby agrees to comply with the shoreland vegetative protection area requirements of Section
4A10 SHORELAND . VEGETATION PROTEC TTON AREA, Sawyer County Zoning Ordinance, in effect on
the date that the land use permit is issued by the zoning office. The owner(s) fii ther agrees to allow authorized
representatives of • the Governmental Unit to enter onto the owner's property at the following description.,
between the hours of 8:00 A.M. and 6:00 P.M. [Section'9.13(1)-POWERS, Sawyer County Zoning Ordinance]
to inspect the structure(s) or uses authorized by said Land Use Permit and to determine if the SHORELINE
VEGBTATION PROTECTION AREA exists and that it is being preserved This authorization shall terminate
on the date of expiration of the land use permit However, the owner's obligation to maintain the SHORELINE
VEGETATION PROTECTION AREA in accordance with Section 4A10 SHORELAND VEGETATION
PROTECTION AREA, Sawyer County Zoning Ordinance remains in effect. Violation of this Agreement will
authorkb, in addition to other penalties and relief required under the Sawyer County Zoning Ordinance,
injunctive relief; restoration of said real estate, the revocation of said Land Use Permit and the removal of all
structures approved by the issue of said Land Use Permit and the revocation of any other uses approved by the
issue of said Land Use Permit. All parties agree that this Agreement shall be filed with the Sawyer County
Zoning Office.
