You must notify Area Water Supply Specialist Jim Hansen at the DNR Area Headquarters, P.O. Box 220, Park Falls, L11 54552, phone 715-762-3204 before starting construction and again not more than 5 days after the project is complete. Finat restoration and re-vegetation of the site is su6ject to the approvaL of Mr. Nansen. 2. You are hereby authorized to place a waste uater outfall constructed of clean fieldstone on the bed and bank cf the river. The rock layer shall extend a maximum distance waterward of 3 feet as measured from the existirg bank. Sufficient bed/bank material shall be removed prior to rotk placement so that the rock layer does nU protrude above the existing bed and bank ccntours. The end of the proposed discharge pipe shall be comptetely covered with rock so that no part of the pipe is visible to river users. 3. The use of filter cloth under the riprap is recortmended to extend the life of the structure and reduce maintenance costs. 4. Removat of vegetative cover and the amount of exposed earth shall be kept m the minimum possible consistent uith good construction practice. Exposed areas shatl be sodded or seeded and mulched as soon as possible to develep vegetative cover. 5. The disturbed corridor shalt be replanted with native shrubs to recreate existing vegetation types and to screen the project from vieu. 6. This permit does not authorize any uork other than uhat is specifically described in your application and p�ans or as may be tonditioned or modified in this permit. Price Rite Inc. has completed filing an application for a permit to place rock riprap atong the shoreline of Namekagon River in the NE1/4 of the NW1/4 of section 33, township 41 north, range 09 west in the Town of Hayward, Sawyer County. 2. The applicant is a riparian owner of the project site. 3. The Department has completed an investigation of the project site and has evaluated the project as described in the application and plans. 4. The proposed project will not materially obstruct navigation and will not be detrimental to other public interests if constructed in accordance with this permit. 5. 7he proposed project uilt not impact wetlands. 6. The Department has determined that the grant or denial of this permit would not be a major state action significantly affecting the quality of the human environment. 7. The Department of Natural Resources and the applicant have completed all procedural requirements and the project as permitted will comply uith all appticable requirements of Section 30. 12(3)(a)3 Wisconsin Statutes and Chapters NR 115, NR 116, NR 177, NR 102, and NR 299 of the Wisconsin Administrative Code. 8. The Department has complied with the requirements of Chapter NR 150 and Section 1 .11 , Wisconsin Statutes. 9. Consistent with social, economic, and other essential considerations, the Department has adopted all practical means to avoid or minimize environmental harm. CONCLUSIONS OF LAW (These are the legal reasons why the Department can issue your permit) 1 . The Department has authority under Section 30. 12(3)(a)3 Wisconsin Statutes and administrative rutes stated above, to issue a permit for the construction and maintenance of this project. 2. The Department has complied with Section 1 .11 , Wisconsin Statutes. 3. The Department has comptied with the requirements of Chapter NR 150 and Section 1 . 11 , Wisconsin Statutes. NOTICE OF APPEAL RIGHTS If you believe that you have a right to challenge this decision, you should know that Wisconsin statutes and administrative rules establish time periods within which requests to review Department decisions must be filed.
