In reviewin�[he petition,it was noted that Ihe request is similar to other petitions accepted by this depar[ment. 2. Based on the precedent established by the previous petitions,this petition for vaziance is being processed as permitted by W isconsin Stamte s.]01.02(6)(g),and Comm 3. Departmental Action:CONDITIONAL APPROVAL Conditions of Approval: 1. AII of the pe[itioner's sta[emen[s included on the variance application(orm,any other documents submitted to the Department,and all conditions of approval,if any,listed below shall be carried out. This variance is specific to the subject petition and cannot be used for any additional modifications. 2. The supporting beams shall be reinforced by doubling the code required beams. 3. An extra 6"X 6"post and pad shall be installed 3'to the north and one to[he east o([he southwest corner post. . �. Care must be taken not to disturb the tank during construction. The tank shall be marked with some sort of barrier to prevent traffic damage. 5. The septic tank shall be inspected to determine that it is in satisfactory condition. This decision will become final unless the department within 30 days from the date of this letter receives a written request for a hearing. A request for hearing should be sent to the address shown on this letterhead. A copy of this letter must be included with the request for a hearing. The request for hearing should state the reasons for objecting to the department's decision, because a request for hearing may be denied if it does not present a significant question in fact, law or policy.
