CONDITIONS: Due to extreme weather conditions, we do not guarantee against concrete cracking. Salt de-icers should not be used. If Excavating is included
in the proposal, back filling is to a rough-grade condition, not landscape grade. Permits are the responsibility of the owner or general contractor unless other
arrangements have been made. We reserve the right to request 50% down payment to schedule a job, if we feel it is needed, at our discretion.
**Concrete work occurring after Odober 15th may be subjed to additional heating/winter charges.

INSP�CTION AND MAIN'I'ENANCE
All components of the stormwater system shall be inspected at least semiannually in carly Spring and early Autumn.
To ensure the proper long-tei7n function of the storm water management practices described above,the following
activities must be completed:
1.All basin overflow must be cllecked periodically to eusure tllere is no blockage from floating debris or ice.Any
blockage must be removed immediately.
2.Inlets and outlets into stonnwater facility must be checked after heavy rains(minimum-annually)for signs of '
erosion.Any eroding areas must be repdired imtnediately to preveut premature sediment build-up.Repaired grass
areas must be restored with a miniinum of 4"of topsoil,seed,fertilizer,and mulch and/or erosion mat. Erosion
matting is recommended for repairing grassed areas. '
3. When sediment has accumulated on the basin bottom which inhibits infiltration of the stormwater facility the
sediment must be removed.
4.No grading or filling of the basin or bern�other than for sediment removal is allowed,unlcss otherwise approved
by the Town or Cotmty.
5.Periodic mowing of the slopes will encourage rigorous grass cover and allow better inspections for erosion. ,
6.Any other repair or maintenance needed to ensure the continued function of the storm water practices or as
ordered by the Town or County under the provisions listed on this Agreement.
Repairs will be made whenevec the performance of a stormwater control stnicture is compromised.
MOWING/F�RTILIZER&CH�MICAL APPLICATION '
Mowing in buffer areas,basin banks and drainage ways will be minimized in order to maximize filtration of ivnoff,
If occasional mowing is necessary,the mowing heiglit will be no shorter than two inches.
Applications of fertilizers,herbicides,pesticide or other chemical applications are discouraged in buffer areas,on ,
basin banks and along drainagc ways. �

It is the responsibility of oia�iter to maintain inspection and maintenance recoi•ds.
In the event the facility owner fails to perform its obligations under this agreement,the Torov�t or County shall have �
the authority to inspect and maintain all components of the stormwater system. In such an event,all associated costs
will be assessed back as a special charge against the property pursuant to Wisconsin State Statutc.
