Dear Major Liethen:
I have received your land use application for a new vehicle maintenance facility located
in the Town of Hayward.
Before we will be able to approve your application we will need to have a soil test for
the property and a sanitary permit that has received state plan approval.
I have enclosed a list of soil testers in the area and also a list of plumbers who install
septic systems. Any of them could obtain the state plan approval and install the system.
Also included is a copy of our fee schedule. Please submit the required fee of
$12,460.00. ($250.00 base fee plus $5.00 per $1,000.00 cost of construction).

Dear Debra,
Thank you for your letter dated August 4, 1998 regarding our land use permit application for our
new military vehicle maintenance facility in Hayward.
Please be advised that the new facility will be a State-owned building, and that Section 13.48( 13)
of the Wisconsin Statutes (attached) exempts the State from paying any permit fees.
Regarding the septic system, we're planning to install a sanitary sewer lift station with a
connection to the city sewer as a part of this construction project. Therefore, I'm assuming that a
soils test and sanitary permit are not required.
Also, we have State of Wisconsin, Dept. of Commerce approved plumbing plans available if you
would like to see a copy.
Please give me a call at (608) 242-3365 if you need more information before you continue to
process the permit application.
