You are hereby authorized to place clean fieldstone or quarry stone 6 to 24 inches in diameter along oc S lineal feet of the shoreline. The stone may extend up to �, feet waterward as measured from the existing bank.The stone shallfollow the natural contour of the shoreline and shall not be placed higher than�_inches above the ordinary high watermark.The final ripra0 slope shall not exceed 2 feet horizontal to 1 foot vertical. 4. All grading and excavation must be confined to the minimum area necessary for construction and shatl not exceed the maximum area allowed by the local shoreland zoning ordinance or 10,000 square feet,whichever is less. 5, The use of filter cloth,or a filter layer,under the riprap is recommended to extend the life of the structure and to improve effectiveness. 6. The prolect shall be constructed in a manner that prevents upland soil losses and sedimentation of the waterway. Temporary erosion control measures such as mulch,silt fence,and/or straw bales shall be used as needed and shall be put in place prior to any precipitation or if work ceases for longer than 24 consecutive hours. Any upland areas disturbed during construction shall be proDerly stabilized immediately following project completion by seeding and mulching. 7, The removal of trees,shrubs,and other shoreline vegetation shall be kept to a minimum and any vegetation removal above the ordinary high watermark of thewaterway must comply with the shore cover removat requirements of the local county zoning ordinance. g. No waterward extension of the upland property is permitted other than what is reasonably necessary for the placement of riprap to stabilize and protect the existing bank. No fill materiai may be placed in a wetiand or below the ordinary high watermark of any surface water. g. This permit is not valid unless you have obtained all required approvals from othe� LOCAL, STATE and FEDERAL jurisdictions. 10. Th�s oermit exnires on November 1 of the calendar vear followina the date of issuance. No construction may take place after that date unless you obtain a wntten extension of this permit from the Department before the date the permit expires. 11. You must keep a copy of this permit at the project site at all times until the project is completed. �Z. You must allow Department employees access to the project site to make inspections at any time to determine whether the work is in compliance with this permit and any other legal requirements. 13. The pre-addressed cazd provided can be used to notify Mr. Chris Wunrow or he can be called at 715-266-3511. 2. This permit expires on November 1, 1998. The placement of topsoil, and the seeding and mulching of the graded azea must be accomplished by this day. No eoustruction may continue after this date unless a new permit or permit extension is gran[ed in writing by che Department. 3. This permit does noc authorize any work other than what you specifically describe in the application and plans, and as limited by condi�ions of this permit. No changes in the project or plans may be made without priot written approval of the Deparnnent. 4. All erosion control measures tha�were indicated to you in a letter from Dan Houston dated May 15, 1998, must be maintained in fu�ccional condition until the e�tire azea is stabilized. 5. The area adjacenc ro the driveway will be replanted with native species of trees in order to blend in with the surrounding forested area. 6. The remainina ve�etative buffer strip next to Phipps Lake shall remain intact. The buffer strip shall not be graded or excavxted. 7. The permittee is respunsible for obtaining any permit or approval required by municipal, town or counry zooing ordinances or hy the U.S. Army Corps of Engineers before completing the project. 8. The permittee shall allow free and unlimited access to the project site at any time to any Department employee who is invescignting [he projecYs construction, operation or maintenauce. 9. The Department may change or revoke this permit if the project obstructs navigation or becomes �etrimental to the public interest. l0. L Edward and Shirlcy Miller have completed filing an application for an after-the-fact permit to grade 17,965 squaze feet on the hxnk of Phipps Lake in the SE1/4 of the SEI/4 of sectio� 36, township 41 north, range 09 west in the Town of Hayward, Sawyer County. 2. The projec� consis�ed �,f grading 17,965 square feet for the purpose of clearing land for a homesite and gazage. A letter dated �lay I5, 1998 to the Millers instructed them to replace the existing filter fence with three tiers of staked straw b<iics backed by tilter fence. Within 3 days of receipt of the letter, the material that had ecoded into Phipps Lake wus cn be removed to the pre-development contours, and the disturbed lake buffer zone seeded and mulched. 3. The Department has completed an investigation of the pcoject site and has eva(uated the project as described in the application and plans. 4. The proposed project will not impact wetlands. 5. The Deparnnent and the applicant have completed all procedural requirements and the project as permitted will comply widi all applicable requirements of Section 30.19(1)(c), Wisconsin Statutes, and Chapters 102, 103, ll5, 116, 150, and hR 299, Wisconsin Administrative Code. 6. The Department has determined that the grant or denial of this permit would not be a major state action significant(y affec�inb [he quality of the human environment.
