Permit is desired for the location of a bulk fuel storage facility (one 30,000 gallon bulk propane tank), by the purchaser. The Town Board approved the application with conditions. The Sawyer County Zoning Committee approved the revised application to include the purchase of two lots (including lot #2 as proposed) and subject to applicant working out an acceptable agreement with the Town's Planning Committee including the drainage issue; also, approval is subject to applicant receiving all the necessary permits from the State. Findings of Fact of the Zoning Committee: It would not be damaging to the rights of others or property values; it has been approved by the Town Board of Hayward subject their conditions. PLEASE NOTE: This approval will expire on August 21,2016 if the permit is not obtained by that date. A PERMIT TO CONSTRUCT OR USE OF LAND IS REQUIRED. NFPA 58,4.4 Qualifications of Personnel. Person who transfer/fill LP containers shall be properly trained with refresher training at least every three years.(Provide training documentation for verification.) 5. NFPA 58, 5.7.4.2 Containers over 4000 gallon shall be equipped in accordance in accordance with 5.7.4.2(A)through G and Table 5.7.4.2) 6. NFPA 58, 5.7.8.5 Container inlet and outlet connections on ASME containers of more than 2000 gallon water capacity shall be labeled to designate whether they communicate with vapor or liquid space. 7. NFPA 58,6.12.10 Emergency shut-offs shall be installed in accordance with this section.All installations shall have at least one clearly identified and easily accessible manually operated remote emergency shutoff device located not less than 25 feet or more than 100 feet in the path of egress from emergency shut-off. 13. NFPA 58,6.13 Hydrostatic relief valve shall be installed in each section of piping and hose in which liquid can be isolated between valves to relieve pressure that may be possible. 14. NFPA 58, 6.18.1 Operations and Maintenance The provisions of Chapter 14 shall apply to all new and existing LPG installations. (Facilities shall prepare and maintain written operating manuals including emergency action for the operator, owner can provide a written statement to inspector at site inspection that manual is completed and will be made available upon request) 15. NFPA 58, 6.18.4.1 Prevent tampering. Fencing or locking devices may be used to prevent tampering or unauthorized persons from entry. (If fenced area over 100 s q ft two means of egress are needed.) 16. NFPA 58,6.25.3.1 Protect ASME Containers and dispensing stations. Fire protection shall be provided for locations with an a�gregate water capacity of more than 4000 gallons. The modes of fire protection shall be specified in a written product release prevention and incident preparedness review. (Owner can provide a written statement to inspector at site inspection that written review is updated and complete and made available upon request) 17. NFPA 58,6.25.4.2 Each industrial plant shall be provided with at least one approved portable fire extin�uisher having a minimum capacity of I 8#of dry chemical with B-C rating. A full size copy of the approved plans, specifications and this letter shall be on-site during construction and open to inspection by authorized representatives of the Department,which may include local inspectors. All permits required by the state or the local municipality shall be obtained prior to commencement of construction/installation/operation. In granting this approval,the Division of Industry Services reserves the right to require changes or additions, should conditions arise making them necessary for code compliance. As per state stats 101.12(2),nothing in this review shall relieve the designer of the responsibility for designin;a safe building, structure,or component.
