Dear Ms Doyle:
John Czarnecki, agent for Pinewood Homes, has requested an amendment to a land use
permit issued to you on March 15, 2004. This permit (# 04-044) was for a new home on
Lot 1 of Certified Survey Map (CSM) recorded in volume 25 page 83.
The requested amendment is to change the location of the dwelling to Lot 1 of CSM
recorded in volume 25 page 94. This change requires a new soil test and sanitary permit
which have been obtained.
Due to recent changes in the zoning ordinance the zoning administrator has instructed me
to revoke the first permit. An application for a new permit has been submitted by Mr.
Czarnecki. We have received the document where you have given him power of attorney
to sign the new application. Enclosed you will find a copy of the revoked permit and a
copy of the new application. The fee for the first permit has been refunded to Pinewood
Homes.

Dear Mr. Bodenschatz;
In March we issued land use permit number 04-044 to Nancy Doyle for a new home on
lot 1 of Certified Survey May volume 25 page 83.
On October 29, 2004 your agent, John Czarnecki, submitted a request to amend that
permit, placing the house on lot 1 of CSM volume 25 page 94. A new soil test and
sanitary permit have been obtained for the new location.
The zoning administrator has instructed me to revoke land use permit# 04-044. We will
refund the $225.00 permit fee. The application for a new land use permit on the other lot
has been received by this office and is being processed. A new fee of$225.00 will be
needed.
Due to changes in the zoning ordinance it has become necessary to have the owner's
signature on all applications. A power-of-attorney from the owner to the builder is also
an option. I have received the document from Nancy Doyle authorizing John Czarnecki to
sign the permit application. In the mean-time I had removed Johns signature from the
application. He will need to stop by the zoning office to re-sign the application.
Please have John stop by to sign the new application and submit a new check. I believe
that will be all we need at this time. If you have any questions please don't hesitate to
call.
